Attention GESD Parents:
Health Exams are required for new scholars entering our district and when entering
Pre-K/Kindergarten, 1st, 3rd, 5th, 7th, 9th and 11th grades. A copy of their physical should be
provided to the school within 30 days. NYS Education Law section 904 allows mandated
physical exams to be done in school if a report from your child’s primary health care provider is
not received.
Please have your child’s healthcare provider fax over a copy of their most recent physical. If
your child does not have a current physical, please schedule an appointment to get one, and let
your scholar’s school nurse know it is scheduled. If we do not receive an up-to-date physical,
one can be done with our school physician before the school year ends. Call your school nurse
with any questions.
